Apicotomy is a surgical intervention performed under local anesthesia, during which the top of the root is cut off, also removing at the same tame the pathological process (purulent formation) that is formed on it. Another name for this intervention is resection of the tip of the tooth root. In this process, formed periapical processes or cysts are usually eliminated. This oral surgery prolongs the life of the teeth and prevents their premature loss.
If conservative treatment, ie canal treatment, does not give results - then surgical intervention (apicotomy) is performed. Thanks to the apicotomy, it is possible for the tooth to still remain in the jaw. This is of great importance for planning some future prosthetic work.
This intervention is performed exclusively on teeth that are "dead", ie on teeth from which the nerve has been removed and appropriate endodontic therapy has been performed.
After giving a local anesthesia surgically, an access incision is formed at the height of the projection of the affected tooth root, ie. the gums are "risen" in the region of the affected tooth, after which special borers access the area around the root tip and it is removed, as well as the chronic periapical process with the affected bone around it. The root canal must be definitely pre-filled. If it is not possible to perform conservative therapy before resection of the root tip,apicotomy can be performed with retrograde filling, with the use of modern materials that serve only for those purposes. After that, the gums are put back in place and sewn up.
First of all, it is necessary to do an adequate X-ray.
